54.

Biotin is one of a group of water soluble vitamins known uscasummer.com as the vitamin B complex. Many of the B vitamins share similar functions in the body, and when the vitamin B complex was first discovered it was mistakenly thought to be a single nutrient.

 55.
Later on, when it was realized that the complex was actually composed of a variety of individual compounds, around twenty nutrients were identified as having vitamin B activity.

 56.
However it now understood that there are only eight members of the B complex that are true vitamins, meaning that they are essential components of the diet.

 57.
The eight B vitamins are:

 58.
Vitamin B1 – Thiamine
59. Vitamin B2 – Riboflavin
60. Vitamin B3 –Nicotinic Acid/Nicotinamide
61. Vitamin B5 – Pantothenic Acid
62. Vitamin B6 – Pyridoxine
63. Vitamin B7 – Biotin
64. Vitamin B9 – Folic Acid
65. Vitamin B12 – Cobalamin.
66. It is difficult to attribute the discovery of biotin to any single scientist, as it was identified and assigned different names several times by a variety of researchers.

 67.
During the early years of the twentieth century different people discovered a growth factor for yeast, and labelled it bios, from the Greek word for life. The same substance was then isolated in the 1940s, and was given the name vitamin H, relating to the German word for skin: Haut.

 68.
At the same time, another team of scientists also isolated the same compound, and assigned it the title of coenzyme R.

 69.
Subsequently it was realised that all these discoveries referred to a single chemical, and finally, forty years after its initial discovery, biotin was unequivocally established as a vitamin in the 1960s.

 70.
Overview of Functions
71. Like all B vitamins, biotin has an important role in converting food into energy. Biotin functions as a cofactor in the intermediary metabolism of macronutrients, meaning it is needed by the human body to process proteins, carbohydrates and fats.

 72.
Biotin is also involved in cell production and gene expression, and contributes to the regulation of immunological and inflammatory functions in the body. Biotin is also essential for proper embryonic growth and development.

 73.
Deficiency
74. Like most mammals, humans do not have the capacity to the synthesise biotin at the cellular level, and so require an exogenous source of the vitamin. The human intestine is exposed to two sources of biotin:

 75.
Biotin from the diet
76. Biotin produced by normal microflora in the large intestine.
77. Because it is a water-soluble vitamin, the body does not have the capacity to store biotin, and so a regular supply is required to avoid a deficiency of the vitamin. A number of factors exist that can increase a person’s risk of biotin deficiency:

 78.
Prolonged consumption of egg white
79. Total parenteral nutrition without added biotin
80. Smoking
81. Pregnancy
82. Certain liver diseases.
83. Considering that biotin is needed for normal cellular function, growth and development, it is not surprising that a deficiency of the vitamin can result in a range of clinical irregularities.

 84.
Most signs of overt biotin deficiency fall into two categories, dermatological abnormalities and neurological disorders:

 85.
Dermatological Abnormalities
86. Hair loss
87. Scaly red rash around the eyes, nose, mouth and genitals
88. Brittle nails
89. Cracking in the corners of the mouth.
90. Neurological Disorders
91. Depression
92. Lethargy
93. Fatigue
94. Hallucinations
95. Numbness and tinging.
96. In addition to exhibiting the above features, individuals who have functional biotin deficiency, as a result of being born with an inability to properly metabolise the vitamin, may experience seizures and impaired immune function.

 97.
Biotin Requirements
98. In 2014, in response to a request from the European Commission, the European Food safety Authority (EFSA) Panel on Dietetic Products, Nutrition and Allergies (NDA) attempted to derive specific Dietary Reference Values DRVs for biotin.

 99.
Previously, biotin requirements were described by an Acceptable Range of Intakes (15 – 100ug per day for adults) laid out by the Scientific Committee for Food in 1993. The panel concluded that, due to insufficient data on biotin intakes and the consequences to health, it was impossible to establish a DRV for biotin. Instead the panel proposed an Adequate Intake for all population groups, based on the intakes and apparent absence of deficiency observed in European countries.

 100.
The following figures are a summary of the Adequate Intakes for biotin suggested by the EFSA panel:

 101.
7-11 months – 6ug per day
102. 1-3 years – 20ug per day
103. 4-10 years – 25ug per day
104. 11-17 years – 35ug per day
105. 18 years or over (including pregnancy) – 40ug per day
106. During lactation – 45ug per day.
107. Biotin Supplementation
108. The claims that supplement companies can make about their products are regulated by the European Food Safety Authority (EFSA). Based on the premise that they are scientifically substantiated, EFSA permits the following claims to be made for biotin:

 109.
Biotin contributes to the maintenance of normal skin
110. Biotin contributes to the maintenance of normal mucus membranes
111. Biotin contributes the maintenance of normal hair
112. Contribution to normal macronutrient metabolism
113. Contribution to normal psychological functions
114. Contributes to the normal function of the immune system
115. Biotin contributes to normal energy-yielding metabolism.
116. In addition to the claims permitted by EFSA, the National Health Service (NHS) suggests that people who have trouble with brittle nails can take a biotin supplement to help strengthen them.

 117.
Food Sources of Biotin
118. Biotin is found in many foods, either as the free form or bound to dietary proteins. The following list gives some rich sources of biotin and the amount of the nutrient provided by each serving:

 119.
Yeast (7g) – 1.4-14ug
120. Whole wheat bread (1 slice) – 0.02-6ug
121. Cooked egg (1 large) – 13-25ug
122. Cheddar cheese (28g) – 0.4-2ug
123. Cooked liver (85g) – 27-35ug
124. Cooked pork (85g) – 2-4ug
125. Cooked salmon (85g) – 4-5ug
126. Avocado (1 whole) – 2-6ug
127. Raspberries (1 cup) – 0.2-2ug
128. Raw cauliflower (1 cup) – 0.2-4ug.
129. It is worth noting that although eggs are a good source of biotin, it is only the yolk that contains the vitamin. In fact, raw egg white contains an antimicrobial protein called avidin that can bind to biotin and inhibit its absorption. However, avidin becomes denatured and loses this property through exposure to heat, so cooked egg whites have no negative effect on biotin absorption.

 130.
Summary of the Benefits of Biotin
131. Healthy Macronutrient Metabolism
132. Macronutrients are a category of nutrients that includes protein, carbohydrates and fat. Together with the other B vitamins, biotin has roles in the processes that enable the human body to effectively utilise:

 133.
Amino acids from protein for growth and repair
134. Glucose from carbohydrates for energy
135. Fatty acids from fat for insulation and cell structure integrity.
136. Blood Sugar
137. Biotin, especially in combination with chromium, facilitates the activity of insulin, which is one of the major hormones involved in regulating blood glucose levels.

 138.
Biotin inhibits the activity of certain enzymes that stimulate the liver to produce glucose, reducing the amount of sugar that enters the blood stream. This mechanism explains why a deficiency of biotin can lead to elevated blood sugar levels, and impaired glucose tolerance.

 139.
Maintenance of Healthy Skin, Hair and Nails
140. Signs of biotin deficiency can include thinning hair, brittle nails and problem with the skin. Biotin cannot be absorbed though hair or skin, despite being added to products that are applied to these areas, and so must be ingested to have any significant effect. There is some research to suggest that a high dose of biotin can help to strengthen weak hair and nails and this property was first discovered when the vitamin was used to treat brittleness and cracking in the hoofs of horses. Biotin is also a popular supplement with people who want to protect against skin conditions such as acne and dermatitis.

 141.
Brain and Cognitive Function
142. The human brain is dependent on a constant supply of glucose for energy. Under normal circumstances the brain exclusively uses glucose as a fuel source, and unlike other tissues brain cells cannot use fat for energy. Biotin has a key role in producing glucose form other substrates, and so contributes to the proper functioning of the brain. Deficiency of biotin has been associated with depression, lethargy, and hallucinations.

 293.
Pakistan (Muhammad Yasir)

 294.
A big opportunity for Pakistan resides in ushering its micro, small and medium enterprises (MSMEs) into the digital age, as revealed in a new report by the United Nations-based Better Than Cash Alliance, in support of its member, the Government of Pakistan, and the State Bank of Pakistan.

 295.
The fates of small businesses and national economies are intertwined. If one thrives, so does the other. Pakistan’s MSMEs account for 90% of enterprise in the country and nearly half of GDP. Their growth remains perpetually hamstrung by a dependence on cash. This tether to cash means that transactions – and horizons – are inevitably limited.

 296.
The adoption of responsible digital payments for merchants, with their proven ability to boost women’s economic participation and deliver the UN’s Sustainable Development Goals, represents a tremendous pocket of potential growth. Digital payments can be a pathway to solve some of the major economic challenges the country faces.

 297.
Digitizing responsibly boosts financial inclusion and future-proofs enterprises, ensures equitable access to formal financial services, improves workflows, reduces operational costs and mitigates against mobility constraints, especially for women entrepreneurs. Designing digital payments, based on female customer needs, represents a market opportunity of over USD $ 650 million nationally.

 298.
“We commend the State Bank of Pakistan for its bold leadership in driving responsible payment digitization for merchants, especially women merchants. Across Asia, digitizing merchant payments has led to rapid MSME growth and, as businesses grow, so do economies. We agree – now is the time to accelerate financial equality for women merchants in Pakistan, and look forward to our continued partnership.” said Dr. Ruth Goodwin-Groen, Managing Director of the UN-based Better Than Cash Alliance.

 299.
The report indicates that the national adoption of digital payments by 2025 in Pakistan can boost GDP by 7 percent, generate four million jobs and realize an additional USD 263 billion in deposits, as defined by the National Payments System Strategy.

 300.
The report also advises active engagement by all key public and private sector actors to work towards building trust in digital payments, make them affordable for MSMEs, incentivize adoption through measures like lower sales rate tax, alongside other key recommendations.

 420.
Karachi (Muhammad Yasir)

 421.
A multi-faceted study conducted by Dr Asaad Nafees, Assistant Professor, Department of Community Health Sciences at the Aga Khan University (AKU) is the largest of its kind to determine the effectiveness of a simple intervention in reducing cotton dust-related respiratory health effects on textile workers in Pakistan. The study titled MultiTex RCT, in collaboration with the National Heart and Lung Institute (NHLI) and Imperial College London, marked a crucial step towards revolutionizing occupational health interventions for textile workers in Pakistan and other Low- and Middle-Income Countries (LMICs).

 422.
Persistent cotton dust exposure leads to the development of a disease called byssinosis, a disease prevalent among textile workers in LMICs due to limited access to occupational health and safety measures. With an initial cohort of 2031 workers from 38 textile mills in Karachi, the study tested a low-cost intervention package to ascertain if it would mitigate the adverse respiratory effects experienced by these workers. The interventions comprised training in occupational health for all workers and managers, regular refresher sessions, formation of workplace committees to enforce a health and safety plan including wet mopping and safe disposal of cotton dust, and provision of face masks. The results showed clear improvements in respiratory symptoms and lung function of the textile workers.

 423.
Funded by the Wellcome Trust for a period of three years, the findings of this important research can be applied to the larger textile landscape helping workers in Pakistan and elsewhere stay safe and minimize their disease burden. Discussing the study outcomes, Prof Paul Cullinan, formerly of Imperial College London, involved in the study said, “Studies of this caliber on occupational health interventions are extremely rare and this work has promise for large-scale uptake since the multifaceted intervention was designed to fit the local context in Pakistan. There is no obvious reason why it could not be rolled out more widely, nationally, and internationally. In the 200 years we have been faced with byssinosis, this work represents a landmark in its control”. He underlined the importance of these simple yet transformative interventions, advocating for their widespread adoption for the benefit the workers.

 424.
Dr Asaad Nafees, the Principal Investigator, explained that “this trial was a parallel, cluster-randomized controlled study with textile mills as the unit of randomization. It can be replicated across the region to potentially impact respiratory health of all relevant workers, and potentially result in a healthier, more productive workforce.”

 425.
Key findings were recently presented at a seminar, where Engineer Sibtain Mughal, Joint Director Labour (OSH), Government of Sindh, committed the government’s support to the implementation of the health measures recommended by the study. “Workers deserve a high level of care and attention since their health holds significant importance for the industrial productivity and economy of the country.”

 426.
Several manuscripts have been published in high-impact occupational health journals. The study was also recently accepted for publication in the European Respiratory Journal.
