175.

In recognition of National Physician Suicide Awareness Day, The Physicians Foundation, announced the launch of Vital Signs, a campaign to help raise greater awareness about the physician suicide epidemic and to provide physicians the tools to attend to their own wellbeing.

 176.
?One physician commits suicide every day, the highest suicide rate of any profession and more than twice that of the general population,? said Gary Price, M.D., president of The Physicians Foundation. ?These are our mothers and fathers, wives and husbands, sons and daughters, friends and neighbors. They are human beings who deal with emotional and physical pain, and neurocognitive issues, like depression and anxiety, all of which are exacerbated by very stressful careers. We cannot continue to lose so many talented and caring professionals every year.?

 177.
Vital Signs is designed to empower physicians, their colleagues and loved ones, to check in on one another?s wellbeing. Ultimately, this will help destigmatize the negativity around physicians seeking help for their mental health. The campaign uses an educational webpage to help understand the warning signs to look for in someone who may be suicidal. In addition, Vital Signs provides a guide to help start a conversation with a physician about whom a colleague may be concerned.

 178.
?To make progress in addressing physician suicide, we must address the persistent stigma surrounding seeking mental help in the profession and the lack of organizational support for physicians coping with the traumatic events and burdens that take away from the joy of practicing medicine. The reasons for suicide are multifactorial but there is no doubt that long term stress and burnout contribute to a greater risk of suicide among physicians,? noted Dr. Price. ?Vital Signs is an effort to reduce that stigma and to get physicians to check in with one another.?

 179.
The Vital Signs campaign is part of the Foundation?s ongoing commitment to protect physician wellbeing, including addressing burnout, which can have devastating effects on the access, quality and cost of our country?s health care. It?s estimated that one million Americans lose their physician to suicide each year.

 180.
In 2018, the Foundation released results from a nationwide survey, which revealed 78% of physicians experienced burnout due to factors such as loss of clinical autonomy, electronic health record (EHR) challenges and barriers to addressing patients? social needs that are directly impacting their health outcomes. To combat the main drivers of burnout and improve physician wellbeing, the Foundation is focused on:

 230.
Louisiana selects four companies to manage care for state's 1.7 million Medicaid enrollees

 231.

 232.

 233.

Theadvocate.com

 234.
The Louisiana Department of Health has picked four companies it intends to award multi-billion dollar contracts to in exchange for managing care for 1.7 million Medicaid patients in the state.
235.
236. The agency picked AmeriHealth Caritas Louisiana, Community Care Health Plan of Louisiana (Healthy Blue), Humana Health Benefit Plan of Louisiana and United Healthcare Community Plan. The terms of the deals are still being worked out but are expected to begin in January.
237.
238. Managed care companies are tasked with providing Medicaid benefits and services to the 1.7 million adults and children who are covered by the state-sponsored insurance program, which was expanded by Gov. John Bel Edwards to cover nearly half a million more people.
239.
240. Currently, the state contracts with five companies to manage services for most of the state Medicaid population. Those companies are Aetna Better Health, AmeriHealth Caritas Louisiana, Healthy Blue, Louisiana Healthcare Connections and United Healthcare Community Plan.
241.
242. Medicaid paid $7.6 billion to those firms in the 2018 fiscal year. The health department forecast it would spend more than $8 billion in the fiscal year that ended June 30 ? including $4.7 billion for regular Medicaid patients and $3 billion for expansion patients.
243.
244. The existing contracts expire at the end of the year. They were negotiated by the Jindal administration and extended by the Edwards administration.
245.
246. The health department began a bid process in February for the new contracts.
247.
248. Former Gov. Bobby Jindal moved the state to the privatized managed care model in 2012, moving away from the old system of directly reimbursing doctors and hospitals for each service for Medicaid patients.
249.
250. Now, most Medicaid enrollees receive their health care through the new managed care model, though not long-term supports and services. Medicaid pays the managed care organizations, often called MCOs, a monthly fee based on the number of enrollees.
251.
252. If a current Medicaid enrollee has a plan with one of the MCOs that was not selected for the new round of contracts, the enrollee may have to select a new plan, the health department said. The open enrollment process begins in October where members can select new health plans.
253.
254. The contracts are expected to be executed later this month.

 271.
Transitioning the Journal of the LSMS in 2019

 272.

 273.

 274.

For 174 years, the Journal of the Louisiana State Medical Society has been providing valuable scientific articles and information to physicians. Established in 1844, the Journal is one of the oldest periodicals of its kind in the country, featuring scientific information on clinical trials and treatments. Originally founded as the New Orleans Medical and Surgical Journal, the publication was "devoted to the cultivation of medicine and the associated sciences." In 1953, the name was changed to the Journal of the Louisiana State Medical Society. No matter what the name, the Journal is not only true to its original cause, but it has adapted to meet the needs of today?s physician.

 275.

 276.
Continuing a proud tradition, the purposes of the Journal have been to serve as a repository of articles of interest to physicians and to serve as a means of communication among the members of the Louisiana State Medical Society. In keeping with the mission of the Journal to meet the needs of today?s physician, in 2019 and beyond, the Journal will transition to a membership publication for member news and information to keep LSMS members informed with the ongoing issues facing the practice of medicine in Louisiana and away from publishing scientific peer-reviewed medical manuscripts indexed in PubMed.

 277.
Through its 174-year history, the Journal has published some truly remarkable works in the field of medicine. Keeping with trends of other state medical societies, the Journal is no longer indexed in PubMed and will only occasionally publish medical manuscripts from section editors without a PubMed ID number. The Journal transitioned to an electronic-only format in 2013 to strive to retain its status as one of the oldest medical journals in the country. Since then, there has been a shift in trends with respect to medical society publishing and indexing in the PubMed database. As of today, only four other medical societies are indexed in PubMed, including the New England Journal of Medicine.

 278.
Thank you for helping make the Journal of the Louisiana State Medical Society one of the longest running medical journals in the country. We look forward to providing the LSMS membership with a printed publication again in 2019.

 279.
Any manuscripts that have not been accepted for publication into the November / December 2018 issue of the Journal are released to submit to another Journal.

 280.
Thank you for your interest in the Journal.

 295. The Honorable John Bel Edwards
296. Governor of Louisiana
297. Office of Governor John Bel Edwards
299. Baton Rouge, LA 70802
300.
301. Governor Edwards,
302.
303. The Louisiana State Medical Society (LSMS) strongly urges the State of Louisiana to follow in the footsteps of Pennsylvania in removing prior authorization requirements for treatment of substance use disorders. We believe and support all initiatives which will result in streamlining the efforts of saving lives in Louisiana. Louisiana can and should serve as a leader in this effort.

 304.

305. We ask that you take immediate action to remove the barriers of such by eliminating prior authorization requirements relative to opioid treatment in Louisiana.

 306.

307. Governor Wolf of Pennsylvania and his administration announced Friday, October 12, 2018, a landmark agreement with the seven major payers in the Commonwealth to end prior authorization for medication-assisted treatment as well ensure that medication-assisted treatment (MAT) is on the lowest cost-sharing tier. Highlights from the announcement can be found below:

 308.

309. ?It is vital we take all possible steps to make sure patients are receiving the most appropriate treatment for their pain, while at the same time appropriately managing and monitoring the risks associated with opioids,? Department of Health Secretary Dr. Rachel Levine said. ?Medication-assisted treatment is an effective, evidence-based treatment to help those with the disease of addiction to opioids and this step by private insurers allows more people with opioid use disorder to be able to access this form of treatment. Treatment works, and recovery is possible for those who are battling this disease.?

 310.

311. These guidelines apply to individual, small group, and large group fully insured plans. Self-funded plans, where employers provide health care coverage administered by a third party, are regulated by the federal government and are not included in this agreement.

 312.

313. The guidelines implement thresholds for prior authorization for long- and short-acting opioids, morphine milligram equivalents (MME) and exceptions for active cancer, sickle cell crisis, and palliative care/hospice patients. Generally, commercial insurers are requiring prior authorization for all long-acting opioid prescriptions and short-acting opioid prescriptions after seven days. Some insurers are phasing in their alignment with many of the guidelines. Patients should consult with their insurer to find out how these guidelines are being incorporated into their specific health plan.

 314.

315. Providing unrestricted access to all commonly used forms of MAT when patients need it is critical to ensuring any individual struggling with addiction can access the type of treatment that is best for them. Under this agreement, commercial insurers will cover MAT without prior authorization in the following ways:

 316.

317. ? Coverage of at least one Buprenorphine/naloxone combination product
318. ? Coverage of Methadone as MAT
319. ? Coverage of injectable and oral Naltrexone
320.
321. Commercial insurers have also committed to coverage of at least one form of nasal naloxone without quantity limits.

 322.

323. The guidelines also provide that MAT will be covered at the lowest patient cost tier on the plan?s pharmacy benefit, as applicable.

 324.

325. Once again, the LSMS implores your leadership in the State of Louisiana to take immediate action to follow in the footsteps of Pennsylvania in removing prior authorization requirements for treatment of substance use disorders.

 326.

327.
